Although a major aim of the study was to examine societal costs of employing this test, the actual cost of the test to an individual patient is nowhere to be found.  It appears to be in the hundreds of dollars, and there is no sign that Medicare is covering it as of yet.  The release notes that a number of US-based insurance companies have contracted to use the test.

Under the "first do no harm principle," it seems reasonable to forgo mass screening as a public health policy at this point.

p53 is required for the DNA damage-mediated stimulation of LMP1.Whether p53 was required for DNA damage-mediated LMP1 stimulation was examined in the TK6 (TP53+/+) and NH32 (TP53−/−) cell lines. Cells were treated by X-ray irradiation or 5-FU. At the desired time points, cell lysates were collected for Western blot analyses. Following DNA-damaging treatments, p53-expressing cells (TK6) had increased expression of p53 as well as LMP1; however, in p53-negative cells (NH32), LMP1 expression was not significantly changed (Fig. 2C). Furthermore, the real-time PCR assays indicated that X-ray treatment could increase the expression of LMP1 RNA in TK6 cells but not in NH32 cells (Fig. 2D). The data support the idea that p53 is required for DNA damage-mediated stimulation of LMP1 expression.

BACKGROUND: Unexplained chronic cough (UCC) causes significant impairments in quality of life. Effective assessment and treatment approaches are needed for UCC. METHODS: This systematic review of randomized controlled trials (RCTs) asked: What is the efficacy of treatment compared with usual care for cough severity, cough frequency, and cough-related quality of life in patients with UCC? Studies of adults and adolescents aged 12 years with a chronic cough of 8 weeks’ duration that was unexplained after systematic investigation and treatment were included and assessed for relevance and quality. Based on the systematic review, guideline suggestions were developed and voted on by using the American College of Chest Physicians organization methodology. RESULTS: Eleven RCTs and five systematic reviews were included. The 11 RCTs reported data on 570 participants with chronic cough who received a variety of interventions. Study quality was high in 10 RCTs. The studies used an assortment of descriptors and assessments to identify UCC. Although gabapentin and morphine exhibited positive effects on cough-related quality of life, only gabapentin was supported as a treatment recommendation. Studies of inhaled corticosteroids (ICS) were affected by intervention fidelity bias; when this factor was addressed, ICS were found to be ineffective for UCC. Esomeprazole was ineffective for UCC without features of gastroesophageal acid reflux. Studies addressing nonacid gastroesophageal reflux disease were not identified. A multimodality speech pathology intervention improved cough severity. CONCLUSIONS: The evidence supporting the diagnosis and management of UCC is limited. UCC requires further study to establish agreed terminology and the optimal methods of investigation using established criteria for intervention fidelity. Speech pathology-based cough suppression is suggested as a treatment option for UCC. This guideline presents suggestions for diagnosis and treatment based on the best available evidence and identifies gaps in our knowledge as well as areas for future research. BACKGROUND: Protracted bacterial bronchitis (PBB) is a common and treatable cause of chronic wet cough in children in which the mechanisms are not understood. Th is study investigates the IL-1 pathway and a neutrophil gene expression signature in PBB.METHODS: BAL was collected from children in an experimental cohort (n = 21, PBB; n = 33, control subjects), and a second validation cohort (n = 36, PBB; n = 11, control subjects). IL-1ß , IL-1 receptor antagonist (IL-1RA), and a -defensins 1-3 were assayed by enzyme-linked immunosorbent assay, western blot, and quantitative real-time polymerase chain reaction, together with selected IL-1 pathway members and neutrophil-related molecules.RESULTS: In the experimental cohort, children with symptomatic PBB had significantly higher levels of IL-1ß and a -defensin gene and protein expression. Expression of the neutrophil chemokine receptor C-X-C motif receptor 2 was also higher in PBB. IL-1RA protein was higher, however, the IL-1RA:IL-1ß ratio was lower in children with PBB than control subjects. In the validation cohort, protein and gene expression of IL-1ß and a -defensins 1-3 were confirmed higher, as was gene expression of IL-1 pathway members and C-X-C motif receptor 2. IL-1ß and a -defensin 1-3 levels lowered when PBB was treated and resolved. In children with recurrent PBB, gene expression of the IL-1ß signaling molecules pellino-1 and IL-1 receptor-associated kinase 2 was significantly higher. IL-1ß protein levels correlated with BAL neutrophilia and the duration and severity of cough symptoms. IL-1ß and a -defensin 1-3 levels were highly correlated.CONCLUSIONS: PBB is characterized by increased IL-1ß pathway activation. IL-1ß and related mediators were associated with BAL neutrophils, cough symptoms, and disease recurrence, providing insight into PBB pathogenesis.
