© 2018 American Academy of Allergy, Asthma & Immunology Background: Chronic refractory cough (CRC), a phenotype of cough hypersensitivity syndrome (CHS), is a disabling problem. Laryngeal dysfunction may be important in CRC and CHS because laryngeal symptoms are common; however, the role of laryngeal dysfunction in CHS has not been systematically examined. Objective: To determine the nature of laryngeal dysfunction in patients with CRC and compare with the related laryngeal conditions of vocal cord dysfunction (VCD) and muscle tension dysphonia (MTD). Methods: A cross-sectional analytic design was used. We recruited 69 participants including healthy controls and patients with CRC, VCD, and MTD who were referred for behavioral speech interventions. Participants underwent a comprehensive assessment of laryngeal function during breathing, phonation, and swallowing. Results: Cough frequency was high in patients with CRC (10.2 coughs/h) and VCD (16.5 coughs/h), but low in healthy controls (1.5 coughs/h) (P <.001). Patients with CRC, VCD, and MTD had impaired voice-related quality of life (vs controls, P <.05) and laryngeal hypersensitivity (vs controls, P <.05). Most voice assessment measures (3 out of 4) were significantly impaired in the CRC group compared with controls and were similar to the VCD and MTD groups. Paradoxical vocal fold movement during respiration was present in 47% of the patients with CRC at rest and in 67% after odor challenge. Mediolateral laryngeal constriction during phonation was present in 45% of the participants with CRC, 93% of the participants with VCD (P <.001 vs CC), and 64% of the participants with MTD. Conclusions: Laryngeal dysfunction is common in CRC and CHS and may contribute to CHS mechanisms. Assessment and treatment of laryngeal dysfunction using speech pathology interventions are likely to be beneficial in CHS.

This study demonstrated that treatment with lactatic acid resulted in lower pH levels following ischemia that approximated that of animals undergoing mechanical postconditioning. Additionally, the level of ROS, as shown indirectly through measurements of MDA and SOD activity, in the Hyd group was comparable to that of the M-Post group. Treatment with lactic acid was able to extend the period of acidosis in ischemic tissue, but did not significantly influence the generation of ROS. In this study, a trend toward a reduction in ROS was also observed; Nonetheless, this difference was not significant. However, other investigators have reported that lactic acid may reduce ROS39,40. This discrepancy may be due to the use of a surrogate marker for ROS production as opposed to more direct measurements of ROS. These data indicate that the combination of hydrogen rich saline and lactic acid provide a way to achieve the degree of acidosis and ROS generation that are hypothesized to trigger postconditioning.
Rats were anesthetized with sodium pentobarbital (46 mg/kg, intraperitoneal injection) and were then intubated and ventilated using a rodent respirator (ventilation rate: 52 breaths per minute, tidal volume: 4.0 mL/100 g body weight). The left pleural cavity was opened, and a 5–0 ligature was placed under the left coronary artery (LCA) by inserting the thread into the left atrium and threading it out from the side of the pulmonary artery cone. A balloon (Grip, 3.0a12 mm, Acrostak Corp., Switzerland) connected to a pump full of water at a pressure of 1 atmosphere (ATM) was placed into the artery to achieve fast reperfusion and ischemia. After securing a knot, the pressure of the balloon was immediately adjusted to 12 ATM for 45 minutes of ischemia. Following the experimental protocols below, after 45 minutes occlusion, the pressure of the balloon was quickly adjusted to zero for various time periods (Fig. 1).

Our results obtained with splenocytes and PBMCs stimulated with C. albicans in vitro (Fig. 1) and previous reports suggest a role of LMP7 inhibition in T helper cell differentiation9,11,13. Thus, we intended to investigate whether the immunoproteasome has an influence on the generation or activation of IL-17A- and IFN-γ-producing cells in response to systemic C. albicans infection in vivo. Analyzing cytokine production of splenocytes upon ex vivo restimulation with heat-killed C. albicans on day 7 postinfection revealed a marked reduction of IL-17A and IFN-γ production upon LMP7 inhibition in vivo (Fig. 2). While immunoproteasome-deficient mice showed no altered susceptibility to invasive candidiasis (Fig. 3A), ONX 0914 treated mice suffered from accelerated and more pronounced weight loss as well as higher mortality compared to vehicle treated mice already on day 2 of infection (Fig. 3B,C). Moreover, in some experiments, we observed that LMP7 inhibition led to movement disorders and neurological abnormalities of mice as manifested by a slight tilting of the head and uncontrolled twisting/rotation when handled (data not shown).
